Instructions

To receive CME credit you must answer ALL questions in the online application. Do not skip any questions; however, comments are optional. The application MUST be completed in one session since your answers will not be saved until you click "SUBMIT" at the bottom of the online application.

Please evaluate each session as to how successfully it met the stated educational objectives. If you did not attend a particular session, select "0" hours attended and "Did not attend" as the evaluation in the second column.

The deadline to submit your application for CME credit is June 1, 2018.

Application Fees

• ASIP members in good standing: There is NO fee to apply for CME credit however; you must register: https://asip.memberclicks.net/index.php?option=com_mc&view=mc&mcid=form_251260
• Non-members: There is a $100 CME application fee. Please select one of the 3 options below:
   
  1. Apply for ASIP membership. Please click on the following link: http://www.asip.org/apply/categories.cfm to read about the ASIP membership categories and then complete the appropriate membership application prior to submitting your CME application form. You will be notified as soon as your application has been reviewed and accepted.
  2. Submit your CME fee online. Please click on the following link to register and pay: https://asip.memberclicks.net/index.php?option=com_mc&view=mc&mcid=form_251260
  3. Submit your $100 fee by completing a PDF of the CME Payment Form. Please navigate to the ASIP meeting website and click on the following link http://asip18.asip.org/cme/cme-payment-form-non-members/. Please return your completed form to the ASIP Education Office as per the instructions on the form.
